![]() Violation Observed: Ensure that the center has a table of organization that illustrates lines of authority, responsibility and communication. 3A:52-4.5(a)(3)(i)(1) The sponsor or sponsor representative shall designate individuals with the authority and responsibility to develop and implement written policies and procedures for the operation of the center, including a table of organization that illustrates lines of authority, responsibility and communication. Violation Observed: Maintain an illness log on file at the center and ensure that it contains all of the required components. 3A:52-7.9(a)(1) For early childhood programs, the following shall apply: the center shall maintain on file a log of the initial illnesses, symptoms of illness, or diseases that are exhibited by each child while in the center’s care, as specified in N.J.A.C. At the time of the inspection 1 named child 12 months was sleep in a a bouncy chair in room 2. Ensure that all cribs used by the center comply with the CPSC Federal Safety Standards. Violation Observed: Ensure that approved sleeping equipment is provided for each child as required. 3A:52-6.4(b)(1) The center shall provide sleeping equipment for children 12 months of age and younger, the center shall provide for each child a crib, that complies with the Consumer Product Safety Commission's (CPSC) Federal Safety Standards for Full- Size and Non-Full-Size Baby Cribs and 1220, which is incorporated herein by reference, and can be obtained through the CPSC's website at Education-Centers/cribs, playpen, or other Office of Licensing-approved sleeping equipment that meets the following requirement that ![]() Violation Observed: Cease the use of pacifier straps or other types of attachment devices in room 2. 3A:52-6.4(i) Children shall not use pacifiers with straps or other types of attachment devices. Violation Observed: Ensure that each child's bottle is labeled with the child's name and date in room 2. 3A:52-6.3(b)(3)(iv)(1) For early childhood programs, the following shall apply, feeding requirements for centers serving children less than 18 months of age are as follows: for bottles, each child's bottle(s) shall be labeled with the child's name and dated. Violation Observed: Ensure staff is aware of how many children are in their care at all times wherein staff in room 2 were unaware of the number children in their care, having to count. 3A:52-4.3(a)(2) The center shall ensure that all staff members are trained in the method of keeping track of children and know how many children are in their care at all times. Violation Observed: Maintain required staff to meet ratios, there were 3 staff with 13 children ages 0-18 months in Classroom #2, an additional staff is required. 3A:52-4.3(c) The following staff/child ratios shall apply, except as specified in 3A:52-4.3(d) through (f):Age Staff/Child RatioUnder 18 months - 1:418 months up to 2 ½ years - 1:62 ½ years up to 4 years - 1:104 years - 1:125 years and older - 1:15 Violation Observed: Maintain required staff to meet ratios, there were 2 staff with 17 children ages 0-2 yrs old in Classroom #2, 2 additional staff is required. Violation Observed: Ensure that the director is at the center at least 50% of the center's daily operating hours. 3A:52-4.5(c)(1) The director shall be scheduled to work at the center at least 50 percent of the center’s daily operating hours. Violation Observed: Ensure that the group teacher is present at the center for at least 75% of the center's daily operating hours or at least six hours a day, whichever is less.
